The Palliative Care and Symptom Management Services Program
The Palliative Care and Home Hospice Program at Northwestern is one of the pioneer programs in the country. Starting with a home hospice program in 1986 and adding a Palliative Care Consultation service in 1993, the service now sees patients across the Northwestern Medicine continuum of care. The main components of the program include:
The inpatient consult service consists of an attending, rotating palliative care or oncology fellows, residents, and students. We see patients throughout Northwestern Medicine. We are happy to assist in the evaluation and management of any or all of the following issues:
Palliative Care Unit
The PCU team consists of an attending physician, a nurse practitioner, and sometimes a palliative care fellow and medical student. If there are beds available, the goals of care are focused on symptom management, and the transfer is deemed appropriate by the consult service or PCU team, patients can be transferred from anywhere in Feinberg or Prentice. Indications for PCU transfer include:

Northwestern Memorial Hospital Home Hospice Program
The Home Hospice Program will help care for patients within Cook County, either at home or in another facility. To inquire about eligibility or to set up services for a patient, call 312-926-4600 to discuss the case with one of our nurse liaisons.

We work together to relieve and manage your symptoms, coordinate your care and identify supportive services that can help you continue to live well.

Lake Forest Hospital and Westmoreland Nursing Center work with multiple hospices to provide the services our patients and their families need, either in a hospital, nursing home or your own home.

The Peri-operative Service grew out of a general internal medicine clinic in October 2003, with original members Drs. Peter Kallas and Anjali Desai, and Mary Murray, PA. The service expanded and merged with the Division in September 2008. Currently,there are 4 physicians and 1 PA who spend 100% of their time on peri-op, with two additional physicians that split time between peri-op and the other services in the Division. We anticipate further growth in this area.
Patients are referred to the pre-operative outpatient clinic by primary care physicians and surgicalspecialists for evaluation of risk prior to surgery. These patients may have multiple clinic visits prior totheir procedure for medication titration and cardiac testing as necessary. The physician that saw thepatient in the clinic will then make an effort to follow the patient in the hospital after surgery. The perioperativeservices also receives new consults from the surgical services.
